VIP is a basic polypeptide neurotransmitter containing 28 amino acid residues, belonging to the cholecystokinin/glucagon-like peptide family. It is primarily synthesized and secreted by the central nervous system, gastrointestinal ganglia, and immune cells. By binding to VIP receptors (VPAC1, VPAC2, PAC1) on target cell membranes to activate the AC-cAMP signaling pathway. It dilates blood vessels to reduce peripheral resistance, regulates gastrointestinal motility and secretion (stimulating pancreatic juice/bile release while suppressing gastric acid), and participates in immune modulation (inhibiting pro-inflammatory factors and enhancing immune cell activity) and neuroprotection (maintaining neuronal survival and promoting nerve regeneration). It is primarily used to aid in diagnosing gastroenteropancreatic neuroendocrine tumors (e.g., measuring serum VIP to diagnose VIPomas). In respiratory diseases like asthma and COPD, it dilates bronchioles to alleviate symptoms. It also plays a role in immunomodulation for autoimmune diseases such as rheumatoid arthritis and in neuroprotection for Alzheimer's disease.
